Quick Mood Check
PHQ-2 (Patient Health Questionnaire-2)
About this check
The PHQ-2 is a 2-question ultra-short screener — just the first two items of the PHQ-9. A score of 3 or more suggests it is worth taking the full PHQ-9 (Depression Check).
Important: This is the shortest depression screener in routine use. It is deliberately sensitive — a positive result is a nudge to look more carefully, not a diagnosis.
What your score means
Below threshold · score 0–2
Your score is below the cutoff used to flag likely depression. Nothing pressing here.
Positive screen · score 3–6
Your score is at or above the cutoff that suggests depression may be present. The PHQ-2 is intentionally sensitive, so this is an invitation to look more carefully, not a verdict.
